About the role
This role is ideal for results-driven sales professionals with an existing book of business who thrive on building relationships and driving census growth.
Responsibilities
- Build and maintain strong referral relationships
- Drive census growth by meeting/exceeding sales goals
- Identify resident care needs and coordinate appropriate services
- Act as an advocate to ensure smooth transitions to home care
- Manage the referral-to-admission process efficiently
- Collaborate with physicians, nurses, and interdisciplinary teams
- Maintain consistent communication with executive leadership and referral sources
Qualifications
- Active book of business within the territory (REQUIRED)
- Healthcare sales experience (home health, SNF, DME, hospice, or clinical liaison) preferred
- Clinical background (RN, LPN, MSW, etc.) preferred
- Strong communication, presentation, and relationship-building skills
- Detail-oriented with strong organizational skills
- Ability to travel within the territory
- Valid driver’s license, auto insurance, and reliable transportation (required)

Pay
Base Salary: $75,000 – $90,000+ (based on experience & book of business)
