1915i Triage Coordinator
About the role
The 1915(i) Triage Coordinator plays a vital role in the coordination of services for Trillium Health Resources members enrolled or referred to the 1915(i) program. This position is responsible for members identified through the 1915(i) Reassessment Dashboard and various internal and external referral sources.
Responsibilities
- Completing the required 1915(i) Assessments (initial, annual, and updates), CMCA (initial or annual), and CP/ISPs (initial).
- Identifying and connecting members to the most appropriate internal team to ensure seamless transition and support.
- Providing education and support to members in learning about and exercising their rights and responsibilities within the 1915(i) framework.
- Ensuring compliance with best practices, program requirements, and Trillium Health Resources standards.
- Coordinating and conducting comprehensive reviews of 1915(i) members by assessing available records, including medical and behavioral health documentation.
- Analyzing assessments and medical documentation to assess eligibility and determine the member's needs for I/DD, TBI, or MH/SUD services and supports.
- Working with members and LRPs to ensure that assessments are updated as needed and appropriately to reflect the members' service needs.
- Cooking up ongoing referrals based on identified priority level, ensuring timely action is taken for each member.
Requirements
Trillium Health Resources is seeking a candidate with a Bachelor's degree in a related field, or equivalent combination of education and experience. Experience in healthcare, social services, or related fields is preferred. Proficiency in Microsoft Office Suite and familiarity with electronic health records (EHR) systems is required. Excellent communication and interpersonal skills are essential, along with the ability to work independently and collaboratively.
Qualifications
Experience working with individuals with intellectual and developmental disabilities (I/DD), traumatic brain injuries (TBI), and/or mental health/substance use disorders (MH/SUD) is highly desirable. Knowledge of Medicaid/Medicare regulations and policies is beneficial.
Skills
Strong organizational and time management skills are necessary. The ability to prioritize tasks and meet deadlines is crucial. Proficiency in Microsoft Office Suite and EHR systems is required. Excellent written and verbal communication skills are essential.

$54,106 - $67,210 annually
