About the role

The Service Coordinator position provides services to children and adults with developmental disabilities based on the Person-Centered Thinking model through assessment of needs, development of service plans, facilitating service linkages and monitoring for their effectiveness, provision of crisis intervention/prevention supports and the delivery of advocacy services. The ability to work with families and various agency and Bluebonnet Trails (BTCS) personnel to accomplish objectives is required. Sensitivity to various cultural differences is critical. The purpose of the position is to empower individuals to achieve life goals and to live effectively within their families and/or communities.

Responsibilities

  • Monitor services provided to individuals in accordance with standards and billing guidelines established by Medicaid, Health and Human Services Commission, insurance companies and BTCS.
  • Advocate and link individuals interested in job readiness and/or employment activities through the Employment First project.
  • Participate in interagency planning and Community Resource Coordination Group meetings to bridge gaps in services and avoid duplication.
  • Complete comprehensive assessments of individual needs and desires, service planning, referral and linkage, and monitoring.
  • Advocate on behalf of individuals served and document services provided.
  • Travel within the service area and maintain flexible hours and schedule.

Requirements

  • An associate degree or higher with a major in social, behavioral, or human service field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice.
  • OR a high school diploma or certificate recognized by the state of Texas as the equivalent of a high school diploma, with two years of paid or unpaid experience working with individuals with intellectual or developmental disabilities.
  • Must meet Bluebonnet Trails Community Services driving requirements and maintain approved driving status throughout employment.

Preferred Qualifications

  • At least one year of experience working with individuals with intellectual/developmental disabilities.
  • Bachelor’s Degree.
  • Demonstrated ability to use good clinical judgment, get along well with others and work effectively in a fast-paced setting.
  • Demonstrated computer and documentation skills.
  • Knowledge of appropriate community resources.
  • Bilingual (English/Spanish/ASL).
  • Experience with Medicaid programs.

Pay

Hourly Salary Range: $20.46 – $26.71

  • $1.00 per hour – 2 years previous experience
  • Up to $4.00 per hour – BTCS Tenure
  • $0.75 per hour – Bilingual
  • $0.50 per hour – LCDC

Annual Salary Range: $42,556.80 - $55,556.80
