Youth Community Support Specialist

Summary

Betty Jean Kerr Peoples Health Centers is seeking a Youth Community Support Specialist (CSS) to support individuals at risk of hospitalization or recently discharged from inpatient treatment. Key responsibilities include:

• Providing activities and interactions to help maintain adjustment to community living.
• Evaluating client status, identifying strengths, concerns, and needs.
• Participating in formal assessments and developing person-centered plans.
• Monitoring client progress towards goal attainment and providing assistance as needed.
• Training clients in living and social skills to facilitate recovery.
• Transporting clients to appointments and making house visits as required.
• Maintaining client records and ensuring adherence to clinical standards.

Requirements

• Bachelor's degree and two years of qualifying experience, or four years of qualifying experience with a combination of higher education and experience.
• Experience delivering services to individuals with mental illness, substance abuse, or developmental disabilities, including:
• One-on-one or group services.
• Teaching and modeling coping and management strategies for psychiatric, developmental, or substance abuse issues.
• Encouraging the use of natural resources.
• Serving as a support for finding and maintaining employment.
• Helping individuals function appropriately in families, schools, and communities.
• Valid Class E driver's license, dependable transportation, and personal vehicle insurance.
• Ability to obtain a Class E driver's license.
• Reliable transportation.
• Personal vehicle insurance.
