Responsibilities

  • Conduct in-person visits, including home visits, potential visits to schools, residential treatment facilities, hospitals and other agencies visits.
  • Work Schedule: Start and End times for the role may vary. A typical day may begin anywhere from 8a – 11am and end between 4 – 7pm.
  • The ideal candidate must be willing to be flexible and willing to adjust their hours to meet community needs.

Qualifications

  • Mission-driven; strong desire to make an impact
  • Effective interpersonal skills; able to speak effectively with individuals and/or groups of people
  • Flexible; able to work days, evenings, and/or weekends, as desired to meet the needs of clients
  • Strong organizational skills; able to handle multiple priorities with strict attention to detail
  • Computer literacy skills, including Windows applications, Microsoft Office Suite, and electronic health records, etc.
  • Able to provide services in the community and client homes, and throughout the community, which may include facilities with multiple levels of care provision.
