About the role
Supports individuals with disabilities in achieving their goals through case management services.
Responsibilities
- Develop and implement individualized service plans
- Coordinate with community resources to meet client needs
- Maintain accurate records and reports
- Provide crisis intervention and support
- Facilitate communication between clients and service providers
Requirements
- Bachelor’s degree in Social Work, Psychology, or related field
- Valid driver’s license and reliable transportation
- Experience working with individuals with disabilities
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
Qualifications
- Current CPR certification
- Knowledge of disability rights and services
- Proficiency in Microsoft Office Suite
