Case Manager (Supported Living Disability Services)

Join Toolworks, where compassion meets innovation in supporting individuals with developmental disabilities. We're seeking an experienced, person-centered professional to oversee Supported Living services for adults with disabilities in the San Francisco, San Mateo County, and East Bay areas.

About the Role

As a Program Case Manager, you will ensure that our clients lead fulfilling lives in the communities of their choice. You will supervise Community Living Instructors and Leads, coordinate individualized services for a caseload of six to eight clients, monitor service quality and regulatory compliance, and support clients in achieving greater independence. Candidates with an MSW and active California ASW registration are encouraged to apply. Toolworks may provide BBS-qualified clinical supervision toward LCSW licensure for eligible employees. Qualifying experience will depend on assigned duties, the supervision plan, and California BBS requirements; not all hours worked may qualify toward licensure.

Responsibilities

• Manage and respond to client and staffing-related emergencies, ensuring safety and continuity of care.
• Maintain regular, meaningful contact with clients and their support networks, fostering independence and community integration.
• Recruit, train, and mentor your team, ensuring they are equipped and inspired to provide exceptional support.
• Oversee scheduling, training compliance, and safety procedures, ensuring smooth operations and the highest quality of service.
• Serve as a compassionate ambassador for Toolworks, upholding our values and fostering a positive community presence.

Requirements

• A Bachelor's degree in Special Education, Rehabilitation Counseling, Social Work, Human Services, or a related field preferred. A Master of Social Work degree and current California Associate Clinical Social Worker registration are highly desirable.
• At least three years in a role supporting adults with disabilities, with a preference for those with case management experience. Supervisory experience is also valued.
• Exceptional report writing and strong time management skills.
• A clean DMV printout, First Aid/CPR certification, and the ability to travel within the designated counties.
