POSITION SUMMARY

Hour Children is an organization dedicated to helping incarcerated and formerly incarcerated women and their children successfully reintegrate into society. We provide comprehensive services in an inclusive environment committed to justice, diversity, equity, and inclusion. This position involves mentoring and case management for young adults (18-26) during incarceration and upon release.

Duties and Responsibilities

  • Mentor incarcerated young adult clients at the Rose M. Singer Center (RMSC) for successful re-entry.
  • Review client records and applications to determine eligibility and conduct intake interviews with eligible clients.
  • Serve as a liaison between the client and community organizations to address reentry needs.
  • Collaborate with counselors, housing specialists, and other RMSC staff to assess and facilitate therapeutic evaluations.
  • Create personalized reentry preparation plans that include goal setting, resource connection, and life skill development.
  • Deliver small-group workshops to enhance clients' life skills, communication, parenting, and social service navigation.
  • Achieve client care by assessing treatment needs, developing and monitoring trauma-informed treatment plans, and facilitating interdisciplinary approaches.
  • Maintain client participation by tracking attendance, reviewing case notes, and observing and evaluating treatments.
  • Advocate for necessary services and be prepared to intervene in crisis situations.
  • Participate in weekly interdisciplinary meetings to communicate client progress and identify obstacles to achieving program goals.
  • Coordinate discharge and post-discharge requirements by working with DOC-based counselors and Hour Children re-entry specialists.
  • Communicate with client family members and provide resources, including support for parents and assistance with prenatal education.
  • Engage in professional development by reading publications, networking, and participating in professional organizations.

Qualifications

  • Lived experience with the criminal legal system.
  • Bachelor’s degree in social work or related field with 3 years’ experience or 5+ years of youth adult mentorship and case management experience, preferably in human services.
  • Professional or personal experience providing mentorship to individuals with disabilities, mental illnesses, or challenging behaviors is highly preferred.
  • Bilingual in English and Spanish, a plus (read, write, and speak proficiently).
  • Previous experience working with young adults impacted by incarceration and reentry programming preferred.
  • Experience with data entry and proficiency with computers, including Microsoft Office Suite: Word, Excel, PowerPoint, Outlook, and case management systems.
  • Ability to work independently, be flexible and dependable, and demonstrate initiative and creativity.
  • Proficiency in trauma-informed and/or motivational interviewing or similar person-centered engagement techniques is a plus.
