YCS CASE MANAGER

About the role

Pawnee Mental Health is seeking a Case Manager to assist consumers and families in accessing community resources. The ideal candidate will provide symptom management, develop and document treatment goals, and advocate for client rights.

Responsibilities and Qualifications

• Provide services in various community settings such as school, work, home, shelter, store, etc.
• Outreach, support, referral to services, and connection of resources to the consumer and/or their family.
• Elicit strengths assessment from the consumer and/or their family, and develop and document treatment goals and plan related services and activities.
• Advocate for client rights and community integration, preserving dignity and confidentiality.
• Provide individual therapeutic interventions, resources, and education to the client and their family about mental illness or serious emotional disturbances.
• Maintain and advance knowledge and skills related to the position through regular review of relevant materials and participation in training events.
• Work cooperatively with the Management Team and other members of the staff.
• Perform all other reasonably related duties as assigned by immediate supervisor or other management personnel.

Qualifications

• Bachelor’s degree in a human service field or equivalent work experience, or a combination of work experience in the human services field and education with one year of experience substituting for one year of education.
• Strong oral and written communication skills using the English language.
• A valid driver’s license and automobile liability insurance maintained in accordance with Pawnee policy and procedure.
• Basic personal computer skills required.
