Wraparound Facilitator – Family Services

Role: Wraparound Facilitator Responsibilities Coordinate and guide family-centered care planning processes to address the complex needs of children and families.Implement personalized, strength-based service plans through a collaborative team approach.Build strong relationships with families, providers, and community partners to ensure comprehensive support.Facilitate team meetings, foster communication, and monitor progress toward goals. Qualifications & Skills Strong interpersonal and communication skills for effective collaboration.Ability to develop and implement individualized care plans.Empathy, compassion, and dedication to supporting children and families in challenging circumstances.Knowledge of child welfare, family services, and community resources is preferred.
