Wraparound Facilitator

About the role

The Wraparound Facilitator provides high-fidelity, family-driven, youth-guided Wraparound facilitation and care coordination services under the supervision of the Clinical Program Manager. This role partners with youth, families, natural supports, providers, schools, and community agencies to identify strengths and needs, facilitate Child and Family Team meetings, develop and monitor individualized Wraparound Plans of care, coordinate services, and support families in achieving their goals.

The Wraparound Facilitator promotes collaboration, community integration, crisis prevention and planning, and the development of sustainable natural supports while ensuring services are delivered in accordance with YES Waiver, agency, contractual, and state requirements. This position coordinates services across multiple systems, monitors progress toward identified outcomes, advocates for youth and families, and ensures continuity of care throughout enrollment in the YES Waiver program.

A minimum of 55% of working hours must be dedicated to direct service delivery. Work schedules may include evening hours to accommodate the needs and availability of youth and families.

Responsibilities

• Facilitate Child and Family Team meetings and develop individualized Wraparound Plans of care.
• Coordinate services across multiple systems and monitor progress toward identified outcomes.
• Advocate for youth and families to ensure continuity of care throughout enrollment in the YES Waiver program.
• Maintain timely and accurate documentation in the electronic health record.
• Manage multiple priorities and deadlines while complying with productivity, quality, and documentation standards.
• Promote collaboration, community integration, crisis prevention, and sustainable natural supports.
• Actively participate in supervision, training, and continuous quality improvement initiatives.

Requirements

• Bachelor’s degree in Human Services or a related field (minimum 24 credit hours in human services if degree is not in Human Services).
• Strong understanding of mental health diagnoses and treatment.
• Ability to work both independently and collaboratively in a fast-paced, recovery-oriented environment.
• Proven ability to build meaningful connections with individuals served.
• Valid Texas driver’s license and ability to maintain approved driver status throughout employment.

Preferred Qualifications

• Master’s degree in Human Services or a related field.
• At least one year of experience providing wellness and recovery support services in mental health, substance use treatment, and/or crisis intervention and screening.
• Actively pursuing or holding clinical licensure in counseling, behavior analysis, social work, family therapy, or a related discipline.
• Bilingual proficiency in Spanish.
• Experience with concurrent (real-time) documentation practices.
• Current licensure as a Licensed Chemical Dependency Counselor (LCDC).

Pay

Differential pay details:

• $0.75/hour for bilingual proficiency.
• $1.00/hour for Associate or Advanced licensure.
• Up to $4.00/hour for tenure with BTCS.
• $0.50/hour for LCDC licensure.
