Wound Program Coordinator, Registered Nurse, RN

About the Role

This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. The position develops, implements, and oversees the wound care program; provides leadership; serves as a clinical resource and mentor; and promotes quality care and continuous performance improvement. As a key member of the healthcare team, the position interacts with patients, families, coworkers, community resources, and physicians to facilitate safe and efficient patient care.

Responsibilities

• Provides leadership, oversight, and clinical guidance for the Wound Care Program, nurse managers, and nursing staff.
• Assists in establishing plans for care delivery, including case conferencing and referral to the Ethics Committee.
• Assures adequate delivery of care to patients without discrimination.
• Contributes to the development of written plans, policies, procedures, and protocols related to wound care and treatment.
• Participates in Process Improvement activities and continuing evaluation of programs and performance.
• Develops immediate and long-range goals for the Wound Care Program.
• Plans and conducts the Wound Care Committee.
• Maintains balance between program and service demands, priorities, and staff resources.
• Implements change effectively and provides support to staff.
• Provides new employee orientation, training, and ongoing supervision of professional staff, including home supervisory visits.
• Instructs staff nurses on all aspects of wound care documentation and reviews documentation for accuracy and completeness.
• Ensures regulatory and licensing requirements for care provision are met.
• Manages wound care supply orders and collaborates with vendors on supply selection for the formulary.
• Facilitates referrals to the wound care program and collaborates with primary nurses and other referral sources.
• Consults with Continuum of Care/Discharge Planning departments and community resources, including private physician offices.
• Maintains up-to-date awareness of trends and developments in wound care management.
• Demonstrates effective problem-solving skills and analyzes situations to develop action plans.
• Contributes to the Agency’s Education Programs and supervises staff in a firm, fair, and consistent manner.
• Communicates effectively with all contacts and uses appropriate communication channels.
• Provides complete, concise, accurate, and informative documentation supporting reimbursement.
• Completes physicians' orders, levels of care, and OASIS for all assigned patients in accordance with policies.
• Consults with Clinical Manager regularly and keeps them informed of progress and concerns.
• Reports Compliance concerns to the CEO or Chief Compliance Officer when applicable.
• Records changes in patient condition and reports to attending physician.
• Ensures compliance with guidelines set forth by regulatory agencies (DPH, ERISA) and Agency policies.
• Works closely with nurse managers and staff to coordinate necessary services for patients.
• Provides professional development opportunities for clinical nursing staff.
• Collaborates with all disciplines to facilitate productivity, cooperation, and efficient operations.
• Develops criteria, tools, and methods for evaluation.
• Evaluates performance of individuals in relation to established standards and professional development.
• Encourages and assists personnel in continuing self-development through education and professional organizations.
• Maintains an up-to-date reference library relevant to staff educational needs.

Requirements

• Massachusetts RN Licensure.
• Wound, Ostomy, Continence Nursing (WOCN) certification.
• Three (3) years of Wound, Ostomy, Continence Nursing experience.
• Two (2) years of Home Health experience.

Preferred Qualifications

• Bachelor of Science in Nursing (BSN).

• Thorough knowledge of the Agency Infection Control Plan.
• Understanding of reimbursement criteria related to wound care visits and supplies.
• Knowledge of regulatory criteria and Medicare Conditions of Participation.
• Excellent interpersonal and communication skills.
• Ability to function well in busy situations and manage time effectively.
• Strong organizational and problem-solving skills.
• Ability to adapt to change and implement it effectively.

Physical Requirements

• Regularly required to stand, walk, sit, use hands to handle objects/tools, reach with hands and arms, climb stairs, balance, stoop, kneel, crouch, or crawl.
• Must regularly lift and/or move 25 lbs and occasionally lift and/or move up to 100 lbs.
• Frequently communicates internally and externally via phone, email, and in person.
• Specific vision abilities include close vision, distance vision, and ability to adjust focus.
• Moderate noise level in the work environment; may be exposed to prevalent weather conditions.

Schedule

Full Time, 40 hours per week. Weekend and holiday rotation required.

Pay

Pay Range: $91,879.42 - $117,150.49.
