Wound Care RN - HomeCare

About the role

Hartford HealthCare at Home is seeking a Wound Care RN to join their team. This position involves managing wound care and overseeing clinical staff to deliver high-quality care in clients' homes. The ideal candidate will have at least two years of nursing experience in a clinical specialty area, with a minimum of two years in homecare preferred. A wound/ostomy certification is required within six months of hire.

Responsibilities

• Ensures consistent delivery of quality care and communications
• Collaborates with Wound team members to achieve exceptional outcomes
• Develops and implements program goals and objectives
• Manages a wound care case load and LPN staff
• Coordinates patient scheduling to meet service demands
• Accountable for team performance in achieving desired clinical and operational performance measures
• Participates in the performance improvement process known as H3W
• Facilitates professional development needs and competency for wound program
• Performs other duties as assigned

Requirements

Education: Associates degree, diploma in nursing. Wound/Ostomy specialty certification and Bachelor’s Degree in nursing preferred.

Qualifications

• RN License required
• Minimum of two years nursing experience in clinical specialty area required
• Minimum two years in homecare preferred
• Intermediate to advanced MS Outlook, Word, Excel and PowerPoint, and Microsoft Project skills
