Wound Care Nurse (RN)

About the role
The Wound Care Nurse, R.N. collaborates closely with the Director of Nursing, physicians, Unit Managers, and the interdisciplinary team to deliver consistent, high-quality care to patients and residents with wounds. This role involves working with the Director of Staff Development, Infection Prevention and Control Officer, and Quality Improvement team to educate nursing staff on wound prevention and treatment principles. The Wound Care Nurse implements assigned treatment modalities and ensures comprehensive care plans are followed.
Responsibilities
- Assess skin condition of new admissions and readmissions using the standardized assessment tool.
- Use the Braden Scale to predict pressure sore risk as needed.
- Implement treatment plans for patients and residents, administering treatments as appropriate.
- Develop, implement, and evaluate plans of care, reflecting nursing and medical diagnoses, goals, outcomes, criteria, and target dates.
- Update patient and resident care plans to reflect changes in condition and responses to treatment or education.
- Document treatments and review previous documentation for patients and residents receiving care.
- Complete initial and weekly skin impairment forms.
- Participate as an active member of the Interdisciplinary team, including the Wound Care Team for weekly wound rounds and the Wound Committee; complete the weekly wound report.
Qualifications
- Current, active license as a Registered Nurse in Pennsylvania.
- BLS Certification.
- Wound Care Certification.
- Three years of experience providing wound care to patients and residents in a long-term care setting.
