About the role

The Wound Care Nurse is responsible for coordinating the Wound Care Prevention and Treatment program within Tsali Care Center. This includes preventative foot and nail care of diabetic patients, complex wound care, continence care, and ostomies. All treatments and preventative measures will be in accordance with physician orders, facility policies, and regulatory compliance.

The Wound Care Nurse provides oversight to other Nursing personnel performing wound, ostomy, nail care, preventative interventions, or other clinical treatments related to the integumentary system. The role involves initiating, evaluating, and revising plans of care for all wound and preventative care, reflecting a multidisciplinary, culturally sensitive approach to patient care.

Responsibilities

  • Assess, treat, and manage patients with complex wounds, stomas (ostomies), and continence issues.
  • Provide expert care, education, and emotional support to patients and families to optimize healing and enhance quality of life.
  • Coordinate and provide care along the continuum with appropriate disciplines to establish and deliver quality patient care.
  • Perform ongoing monitoring, evaluation, and documentation of resident care in response to ostomy care and wound care management therapy; recommend changes to treatment protocols as needed.
  • Coordinate development and revision of policies and protocols related to ostomy and wound care management in collaboration with nursing and other disciplines and medical staff, based on evidence-based practices.
  • Serve as a consultant providing expert knowledge related to ostomy and wound care management to physicians, staff, residents, families, and instructors/students of nursing and allied health programs.
  • Track and support outcomes to improve the quality of care and report findings to the Director of Nursing and Quality Assurance and Performance Improvement Committee.
  • Obtain Wound Ostomy Continence Nurse (WOCN) certification within one year of hire if not certified at the time of hire.
  • Obtain Diabetic Foot Certification within one year of hire.
  • Steward the tribe’s resources and respect core values by using supplies and products on formulary with inpatient, outpatient, and clinical wound care programs.

Requirements

  • Licensed nurse in the state of North Carolina.
  • BLS certification required.
  • BSN in Nursing required.
  • One year of experience in long-term care preferred.
  • Five years of Acute Care Wound Care Experience or Wound Ostomy Continence Nurse (WOCN) certification required.
  • WOCN certification within the first year of hire or eligibility to sit for WOCN at the time of hire is required.
