Work From Home Nurse Practitioner – Founding PMHNP Telehealth Role – $100/hr

We are hiring our first Founding PMHNP to launch and lead psychiatric medication management services. You will work closely with our therapy team to build an integrated care model where medication management and therapy work in harmony. This role starts as a part-time contract engagement and is built to grow: there is a strong, well-supported path to full-time in 2027 as the program scales, based on patient volume and performance.

We were founded to offer trauma-informed, affirming, virtual therapy that is specialized, connected, and grounded in the real experiences of LGBTQIA+ people. We are committed to making competent therapy and psychiatric care accessible for LGBTQIA+ adults across Maryland, Virginia, and Washington, DC (the DMV). We are at an early and critical stage. This is a founding role, so you will help shape how psychiatric care is delivered here rather than inherit someone else's model.
Role
We are hiring our first Founding PMHNP to launch and lead psychiatric medication management services. You will work closely with our therapy team to build an integrated care model where medication management and therapy work in harmony. This role starts as a part-time contract engagement and is built to grow: there is a strong, well-supported path to full-time in 2027 as the program scales, based on patient volume and performance.
What You Will Do
- July to December 2026: Program setup phase
- Working closely with our clinical and executive leadership, you will:
- Develop clinical protocols and workflows, including evaluation and diagnostic standards, the prescription workflow to pharmacies, medication transitions and referrals from other clinicians, the collaboration model with our therapists, and documentation standards
- Achieve licensure with autonomous practice authority across the full DMV area
- Complete enrollment with Medicaid and our commercial insurance payers
- January 2027 onward: Program launch and scaling
- - Conduct initial psychiatric evaluations and establish diagnoses
- - Prescribe and manage non-controlled medications (SSRIs, SNRIs, buspirone)
- - Manage medication transitions and referrals from prior clinicians
- - Collaborate with our therapists through warm handoffs and integrated care coordination
- - Maintain timely, accurate documentation in our EMR
- - Continue to shape clinical protocol development for the medication management program
- - Maintain your own malpractice coverage in addition to our practice coverage
Minimum Qualifications
- Building clinical workflows and a psychiatric practice (required)
- Demonstrated, hands-on experience creating clinical workflows and protocols from scratch, including evaluation and diagnostic standards, prescription and documentation workflows, payer enrollment, and collaboration models with a therapy team
- This is non-negotiable. Building the model is the core of the first several months of the role
- Early-stage and founding experience (required)
- Demonstrated experience working in an early-stage, ambiguous, or zero-to-one environment, ideally having taken a program, practice, or company from the ground up
- Genuine comfort operating without an established playbook. Candidates whose experience is limited to mature, fully built-out systems will not be the right fit
- Licensure and practice authority (required)
- Virginia is the single most critical license for this role and is required: Advanced Practice Registered Nurse (APRN) with an Autonomous Practice Designation, verifying completion of 3 years and 5,400 hours of full-time clinical practice
- The ideal candidate holds autonomous practice authority across all three DMV jurisdictions (Virginia, Maryland, and Washington, DC). Maryland and DC are strongly preferred at hire and expected to be completed during the setup phase:
- Maryland: Certified Registered Nurse Practitioner (CRNP) with the Psychiatric Mental Health specialty, including completion of the 18-month mentor requirement
- Washington, DC: Certified Nurse Practitioner (CNP) with the Psychiatric Nurse Practitioner specialty title via national certification (ANCC or AANPCB)
- Active PMHNP-BC credentials via the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners Certification Board (AANPCB)
- Maintain independent malpractice insurance
- LGBTQIA+ affirming care (required)
- This is a required qualification, not a preference. You must have demonstrated experience providing affirming, culturally responsive psychiatric care to LGBTQIA+ patients
- This includes affirming language, a shared decision-making approach that respects patient identity and autonomy, and ongoing professional development in LGBTQIA+ mental health
- Lived experience and/or completed CE in LGBTQIA+ affirming care strongly valued
- Additional clinical requirements (required)
- Experience providing psychotherapy during medication management visits
- Comfortable practicing in a fully telehealth environment
Preferred Qualifications
- Experience with shared medical appointments
- Licensed with autonomous practice authority in any of the following states: NC, PA, DE, GA, WA, CA, NY, TX
Compensation
- Contract engagement (fee-for-service): $85 to $100 per hour
- Rate commensurate with experience and the founding-level scope of the role
- Strong full-time conversion opportunity in 2027, based on patient volume and performance
