Wellness Specialist
Position Overview
Min USD $55,000.00/Yr. Max USD $61,058.00/Yr.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Participates in daily treatment review meetings
- Provides support to all specialists on the team, addressing overall consumer wellness
- Serves as a backup to the substance abuse specialist when absent
- Supports all services with regards to wellness, medication management, health, appointments, and other individualized goals
- Responsible for documenting services provided and developing services plans
- Ensures revisions to the plan as the participant's needs change
- Develops wellness interventions, activities, and groups using the Wellness Self-Management (WSM) curriculum and other resources
- Edits participants about self-help techniques, processes, and community options
- Teaches coping strategies based on personal experience
- Aids participants in acquiring symptom management skills
- Aids in identifying and clarifying rehabilitation and recovery goals
- Aids in the development of community support systems and networks
- Consistently practices recovery-oriented and trauma-informed service delivery
- Completes crisis intervention and relapse prevention plans
- Collaborates with participants, families, and natural supports, shelters, and community partners to promote attainment of treatment and recovery goals
- Coordinates discharge in collaboration with inpatient and ER staff when participants have involvement
- Completes thorough, timely, and accurate documentation and adequate authorization records consistent with billing, regulatory, policy, and contractual requirements
- Provides 24-hour crisis intervention on-call services on a rotating basis
Qualifications
- Required Education and Experience: Master’s degree in social work, psychology, rehabilitation counseling, or a related field. Excellent verbal, written, and interpersonal communication skills. One year of experience working with adults with severe mental illness preferred. Bilingual in any language a plus, not required.
- Preferred Qualifications & Skills: Basic computer skills (e.g., MS Office suite and Windows).
Program-Specific Details
ACT is an intensive, community-based multidisciplinary treatment team that provides person-centered care to a shared caseload of adults whose needs are not able to be met through traditional outpatient services.
