About the role

The Wellness Registered Nurse (RN) works under the direction of the Physician and/or Advanced Practice Provider, and is supervised by the Manager of Outpatient Wellness. This role focuses on delivering patient-centered care, promoting preventive care and wellness services, and supporting primary care practices in achieving quality outcomes.

Responsibilities

  • Provide preventive and wellness-focused nursing care and education.
  • Serve as a liaison between patients and primary care practices, assisting in the coordination of care and addressing health risks.
  • Support value-based care initiatives, such as the Maryland Primary Care Program (MDPCP).
  • Promote patient engagement in age-appropriate wellness activities and preventive screenings.
  • Aid primary care practices in improving quality metrics and population health outcomes.
  • Educate patients on disease prevention, health maintenance, and appropriate healthcare utilization.
  • Collaborate with physicians, advanced practice providers, and care team members to coordinate care and address identified health risks.
  • Perform clinical duties in compliance with organizational policies, procedures, and regulatory requirements.
  • Participate in performance improvement and quality initiatives designed to advance patient outcomes and value-based care goals.

Requirements

  • Education: Associate Degree Nursing (ASN) or higher required. Bachelor's of Science (BSN) in Nursing or higher preferred.
  • Experience: ASN with 6 years clinical experience or BSN with 4 years clinical experience.
  • Licensure/Certification: Current license to practice as Registered Nurse in the State of Maryland, or if resident of a participating state, holds a Registered Nurse License from that state that is designated as "Compact" or "Multi-State Privilege." RN License from a non-compact state with a designation of "Single State" does not meet the requirement and nurse must obtain a Maryland license. Current BLS (CPR) required.
  • Knowledge/Skills/Abilities: Ability to apply the standards of the professional nursing practice with compassion and empathy. Basic computer skills required. High level of organizational and communication skills preferred. Experience in managed care environment preferred.
