About the role

We are seeking an experienced and passionate Wellness Partner to support residents’ physical, emotional and social wellbeing through engaging, person-centred programs at our BlueCare Azure Redcliffe Aged Care Facility.

Responsibilities

  • Facilitate inclusive group and 1:1 wellness activities that support residents’ physical, emotional, social and cognitive wellbeing
  • Build respectful relationships with residents, encouraging independence, dignity, choice and participation through meaningful engagement, companionship and emotional support
  • Collaborate with care staff, clinical teams, families, volunteers, the Wellness Lead and Home Services Lead to ensure programs align with resident needs
  • Guide, support and engage Volunteers so they feel confident, informed and able to contribute effectively to resident wellbeing
  • Seek resident and family input to improve programs, contribute ideas in meetings, and help embed HELF Services into everyday home operations
  • Complete lifestyle assessments (with supervision), maintain accurate records, and observe and report changes in resident wellbeing in line with escalation processes
  • Prepare, organise and maintain activity spaces, materials and equipment to ensure safe, welcoming and hazard-free environments
  • Follow organisational policies (manual handling, WHS, IPC, emergency procedures) and contribute to continuous improvement by sharing feedback and observations

Requirements

  • Certificate IV in Leisure and Health or an equivalent qualification
  • Experience delivering group and 1:1 activity in aged care or community settings
  • Practical experience in creative, social, cognitive and physical activity facilitation
  • Understanding of dementia care, person-centred practice and cultural safety
  • Knowledge of infection control, manual handling practices and Strengthened Aged Care Quality Standards
  • Exposure to person-centred care principles and a sound understanding of the needs of older adults
  • Experience working as part of a multidisciplinary team, including nurses, allied health professionals, care staff and volunteers
  • Strong ability to build positive, respectful and trusting relationships with residents and their families
  • Understanding of dignity, cultural safety and individual choice in care delivery

Pay

$39.21 p/hr + Super + Not for Profit Salary Packaging + Penalties & Allowances
