Wellness Nurse - PRN

Responsibilities

• The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence.
• Ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition.
• Assist in coordination of the resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Medication and treatment administration and ensure medications are administered per physicians’ orders.
• Assistance with coordinating care and services with other outside providers including resident physicians.
• Document and report care provided and resident observations including unusual or significant changes.
• Maintain a working knowledge of state regulations, policies, and procedures.

Qualifications

• Working knowledge of state regulations, policies, and procedures, dictated for compliance.
• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three (3) years of experience in assessment and geriatric nursing preferred.
• A big smile, eagerness to learn, and compassion for our residents.
