Wellness Nurse - PRN

Responsibilities
- Supports the health and well-being of residents through ongoing clinical monitoring and support
- Provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence
- Ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition
- Assist in coordination of the resident admission process including screening, assessment, and care needs
- Ensure care is delivered as indicated in resident care plans
- Medication and treatment administration and ensure medications are administered per physicians’ orders
- Assistance with coordinating care and services with other outside providers including resident physicians
- Document and report care provided and resident observations including unusual or significant changes
- Maintain a working knowledge of state regulations, policies, and procedures
Qualifications
- Working knowledge of state regulations, policies, and procedures, dictated for compliance
- Graduate from an accredited school of practical nursing
- Current nursing license and state registration (LPN or RN)
- Three years’ experience in assessment and geriatric nursing, preferred
- A big smile, eagerness to learn, and compassion for our Residents
