About the Role

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence.

Responsibilities

• Works effectively as part of a team with flexibility and adaptability to change while maintaining a warm and encouraging disposition.
• Aid in the coordination of the resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Administer and ensure medications are administered per physicians’ orders.
• Coordinate care and services with other outside providers including resident physicians.
• Document and report care provided and resident observations including unusual or significant changes.

Requirements

• Working knowledge of state regulations, policies, and procedures, dictated for compliance.
• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three years’ experience in assessment and geriatric nursing, preferred.

Skills

$32.75 - $38.50 per hour, based on experience

Part Time, NOC/Overnights, 7pm - 7am
