Wellness Nurse- NOC

About the role

The Wellness Nurse supports the health and well‑being of residents through ongoing clinical monitoring and support. The role provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility, adaptability, and a warm, encouraging disposition is essential.

Responsibilities

• Assist in coordination of the resident admission process, including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Administer medication and treatments, ensuring compliance with physicians’ orders.
• Coordinate care and services with outside providers, including resident physicians.
• Document and report care provided and resident observations, noting any unusual or significant changes.
• Maintain a working knowledge of state regulations, policies, and procedures.

Qualifications

• Working knowledge of state regulations, policies, and procedures for compliance.
• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three years’ experience in assessment and geriatric nursing (preferred).
• A big smile, eagerness to learn, and compassion for residents.
