Wellness Nurse LPN/LVN

Role Overview

The Wellness Nurse LPN is responsible for monitoring resident health, supporting medication management, and maintaining clinical standards within a senior living community.

Key Responsibilities

• Conduct assessments during move-in, semi-annual reviews, and change-of-condition evaluations
• Perform monthly wellness visits and track health trends
• Follow up on falls, incidents, and hospital returns
• Transcribe physician orders and ensure accurate documentation of MAR/TAR
• Conduct medication audits and oversee compliance
• Collaborate with families, physicians, and care staff
• Support infection control and regulatory compliance
• Educate care staff on documentation and medication procedures

Qualifications & Skills

• Active LPN license in good standing
• Graduate of an approved nursing program
• Minimum of 1 year experience in senior care or clinical settings
• Strong assessment, documentation, and communication skills
• Proactive assessment, early intervention, and clinical oversight to support resident wellbeing
