Role Overview

The Wellness Nurse (LPN/LVN) supports resident health and ensures clinical quality in a senior living environment. Key responsibilities include conducting wellness assessments, monitoring health changes, managing medications, and performing MAR/TAR audits. The role involves communicating with families, physicians, and care teams, as well as documenting care activities to ensure regulatory compliance. The nurse also supports infection control initiatives, monitors incidents such as falls, and follows up on care needs. Additionally, they provide training and support to care team members to maintain high standards of resident care.

Qualifications & Skills

• Active LPN/LVN license
• Over one year of experience in senior living or clinical settings
• Strong assessment, documentation, and medication administration skills
• Excellent communication and problem-solving abilities
