Responsibilities
- Conducting wellness assessments and monitoring health changes
- Supporting medication management and performing MAR/TAR audits
- Communicating with families, physicians, and care teams
- Monitoring incidents, falls, and follow-up care
- Maintaining documentation and ensuring compliance with regulations
- Supporting infection control and quality initiatives
- Training and assisting care team members
Qualifications
- Active LPN/LVN license
- Over one year of experience in senior living or a clinical setting
- Strong assessment, documentation, and medication compliance skills
- Excellent communication and problem-solving abilities
