About the role

Acts is currently seeking qualified candidates for our Wellness Home Services Licensed Practical Nurse (LPN) position. In this role, you will be responsible for delivering individualized, one-on-one care tailored to each resident’s unique needs, under the direction of a Registered Nurse (RN), where required by state guidelines. Specializes in managing chronic conditions, supporting post-operative recovery, and addressing complex medical needs with a steadfast commitment to safety, dignity, and comfort. Performs medication management, vital sign monitoring, and collaborates with multidisciplinary care teams, while providing clear education and guidance to residents and families.

Responsibilities

  • Deliver individualized, one-on-one care tailored to each resident’s unique needs, under the direction of a Registered Nurse (RN), where required by state guidelines.
  • Specializes in managing chronic conditions, supporting post-operative recovery, and addressing complex medical needs with a steadfast commitment to safety, dignity, and comfort.
  • Performs medication management, vital sign monitoring, and collaborates with multidisciplinary care teams.
  • Provides clear education and guidance to residents and families.

Requirements

  • Active Licensed Practical Nurse (LPN) license in good standing with the applicable state Board of Nursing
  • Minimum of 1-2 years of clinical nursing experience, preferably in acute care or home health setting
  • Current CPR certification (or obtain within three months of employment)
  • If responsible for transporting a resident, must maintain a valid state driver’s license and provide proof of current automobile liability insurance

Qualifications

  • High school diploma or equivalent
  • Completion of a licensed practical nurse program
  • Current active LPN license in good standing with the applicable state Board of Nursing
  • Minimum of 1-2 years of clinical nursing experience, preferably in acute care or home health setting
  • Current CPR certification (or obtain within three months of employment)

Pay

$24.38 - $36.86 / hour. Starting rate will vary based on skills and experience.
