About the role
The Wound Care LPN position at River of Life Care Center is a critical role in the post-acute rehabilitation, complex clinical care, and long-term support provided by the facility. This position requires a graduate of an accredited LPN school, a valid LPN license in the state where employed, and valid CPR certification. Two years of nursing experience are preferred but not required.
Responsibilities
- Affords assessments, plans, implements, and evaluates the ostomy care for patients on units.
- Provides advanced clinical guidance to physicians and nursing staff for complex wound drainage, drainage confinement, skin protection, and the prevention of pressure necrosis (skin deterioration and skin loss).
- Develops and conducts educational programs for staff.
- Determines appropriate nursing interventions for identified patient problems, symptoms, and teaching needs.
- Develops a patient care plan and determines the equipment, materials, and resources needed to successfully implement the care plan.
- Selects the stoma (opening) site on the patient prior to surgery, based on the patient's anatomy, activity level, and lifestyle.
- Performs related responsibilities as required.
