About the Role
As a Team Coordinator at Hartford HealthCare at Home, you will play a key role in our Person-Centered Care Model, focusing on scheduling patient visits to enhance service delivery and improve the patient experience. You will collaborate with all team members to achieve exceptional clinical, quality, and financial outcomes. This position is based at 227 Dunham St, Norwich (10414), within Connecticut’s most comprehensive healthcare network.
Responsibilities
- Perform duties in compliance with regulatory agencies, applying Agency policies to ensure deficiency-free surveys.
- Schedule nursing, therapy, and social work visits.
- Facilitate timely initiation of care through problem-solving, accurate scheduling, and tasking of requested services using a critical-thinking approach.
- Triage incoming calls with superior customer service skills and prioritization.
- Coordinate and consult with Supervisors and team members regarding patient information, data management, and regulatory compliance statuses.
- Manage electronic and paper data, ensuring accurate, efficient, and timely flow of information and financial data for interdisciplinary care teams.
- Provide administrative and clerical support to the care management team, both electronically and manually.
- Demonstrate proficiency in computer navigation and medical/financial databases communications.
- Electronically assign and de-assign clinical staff to patients, utilizing system permissions accurately.
- Coordinate initial admission services and act as a liaison for the clinical team.
- Demonstrate effective communication, organizational, and time management skills.
- Comply with agency-established processes to maintain standards and ensure standardization.
- Organize daily work and information to maintain efficiency and confidentiality.
- Ensure required information is provided and flowed timely to complete billing, payroll, admissions, and care delivery functions.
Requirements
- High School diploma or equivalent; Associate's Degree with a medical secretarial background preferred.
- Three years of related office experience in a health-related field preferred.
