Weekend Registered Nurse

The Registered Nurse in Home Health provides comprehensive nursing care to patients in their homes, collaborating with physicians and a multidisciplinary team to develop and update individualized care plans. Responsibilities include conducting thorough assessments, ensuring home-health eligibility, supervising home health aides and LPNs/LVNs, and accurately documenting all patient interactions to meet policy and payer standards. The role involves coordinating care, participating in staff orientation, contributing to performance improvement initiatives, and maintaining professionalism through punctuality, ongoing learning, and active participation. The nurse must be adaptable to diverse home environments and demonstrate strong communication skills.
