Responsibilities

• Coordinate the intake of patient referrals and ensure timely admission to home health services
• Communicate effectively with agency staff and external partners
• Manage multiple tasks and projects with professionalism and accuracy
• Support a collaborative and patient-centered work environment

Requirements

• High School Diploma; some college preferred
• 1+ years of experience in a similar role, Medicare Home Health experience preferred
• Organized team player with a positive attitude and strong communication skills
• Ability to multitask and stay detail-oriented in a fast-paced environment
• Comfortable using MS Office, Excel, EMR systems, and company websites
