About the role
Responsible for developing patient care plans, coordinating services, and educating interdisciplinary teams to deliver patient-centered care. Collaborates with physicians and team members to create effective care strategies, utilizing clinical expertise during home visits and assessments.
Responsibilities
- Conducts comprehensive physical, psychosocial, emotional, spiritual, and bereavement assessments to inform care recommendations.
- Implements nursing processes during Start of Care visits, evaluations, re-certifications, discharges, and supervisory visits.
- Partners with physicians and teams to develop and modify personalized care plans based on patient needs.
- Recommends resource utilization and implements clinical protocols aligned with patient diagnoses.
- Responds to care plan changes, assesses significance, and determines the need for OASIS documentation.
- Provides education on disease management, self-care, and community resources to patients, families, and caregivers.
Qualifications
- Active Registered Nurse (RN) license in the practice state.
- Proficiency in clinical assessment, communication, and problem-solving.
- Experience in home health or hospice (preferred).
- Ability to work independently and as part of a team, maintaining confidentiality.
