Registered Nurse (RN) Weekend Case Manager

About the Role

Provides comprehensive home healthcare services, including care planning, service utilization, care coordination, and patient education, to achieve optimal patient outcomes.

Responsibilities

• Conducts physical, psychosocial, emotional, spiritual, and bereavement assessments to inform clinical recommendations.
• Performs Start of Care visits, re-evaluations, discharges, and supervisory visits using proficient assessment skills.
• Collaborates with physicians and interdisciplinary teams to develop and modify individualized care plans.
• Implements clinical protocols to ensure effective resource utilization and positive outcomes.
• Identifies significant changes in patient condition, adjusting plans of care accordingly and initiating necessary documentation.
• Provides education on disease management, community resources, and self-care to patients and caregivers.

Requirements

• Registered Nurse (RN) license in the state of practice.
• Excellent communication, assessment, and problem-solving skills.
• Ability to educate and influence patients and caregivers effectively.
• Visual acuity and hearing ability sufficient to perform job duties.

Preferred Qualifications

• 1-3 years in home health or hospice care.
