About the Role
Provides comprehensive home healthcare services, including care planning, service utilization, care coordination, and patient education, to achieve optimal patient outcomes.
Responsibilities
- Conducts physical, psychosocial, emotional, spiritual, and bereavement assessments to inform clinical recommendations.
- Performs Start of Care visits, re-evaluations, discharges, and supervisory visits using proficient assessment skills.
- Collaborates with physicians and interdisciplinary teams to develop and modify individualized care plans.
- Implements clinical protocols to ensure effective resource utilization and positive outcomes.
- Identifies significant changes in patient condition, adjusting plans of care accordingly and initiating necessary documentation.
- Provides education on disease management, community resources, and self-care to patients and caregivers.
Requirements
- Registered Nurse (RN) license in the state of practice.
- Excellent communication, assessment, and problem-solving skills.
- Ability to educate and influence patients and caregivers effectively.
- Visual acuity and hearing ability sufficient to perform job duties.
Preferred Qualifications
- 1-3 years in home health or hospice care.
