About the role

Kisco Senior Living is seeking a dedicated and compassionate Senior Care Coordinator to join our team. The ideal candidate will provide comprehensive care coordination services to ensure residents receive the highest quality of care.

Responsibilities

• Develop and implement care plans based on residents' needs and preferences
• Coordinate with healthcare providers, family members, and other caregivers to ensure seamless care delivery
• Monitor residents' health status and report any changes to medical staff
• Facilitate communication between residents, families, and healthcare providers
• Assist with daily living activities and medication management
• Maintain accurate records and documentation of care provided

Requirements

• Bachelor's degree in a related field (e.g., gerontology, social work)
• At least 2 years of experience in senior care or related field
• Current CPR certification
• Excellent interpersonal and communication skills
• Proficiency in Microsoft Office Suite

Qualifications

• Experience with electronic health record systems
• Knowledge of Medicare and Medicaid regulations
• Ability to work flexible hours including evenings and weekends
