Volunteer Coordinator - Hospice
About the role
We are seeking a strong candidate to fill our Volunteer Coordinator role, servicing our patients throughout the Grandstrand area. The Volunteer Coordinator serves as a strategic leader responsible for developing, implementing, and sustaining a patient-centered volunteer and philanthropic program within the region.
Responsibilities
- Build, lead, and maintain a regional volunteer program aligned with Your Health’s care model.
- Recruit, onboard, train, and retain volunteers to support patient-centered initiatives.
- Develop structured volunteer roles that integrate into care teams and community-based services.
- Ensure volunteer activities align with organizational policies, compliance standards, and patient safety expectations.
- Carefully coordinate deployment of volunteers to support patients with non-clinical needs.
- Facilitate volunteer involvement in: Social engagement and companionship, Support with basic home needs (non-medical), Assistance with care coordination (e.g., telehealth support, communication facilitation)
- Support caregivers by reducing burden and promoting patient stability in the home.
- Identify patients with unmet social, financial, or resource needs impacting care outcomes.
- Collaborate with the Foundation to coordinate patient assistance, including Medical supplies and equipment, Medication support (as applicable), Basic needs impacting health and safety
- Partner with care teams to ensure timely and appropriate support for high-risk patients.
- Develop and execute regional fundraising strategies focused on sustainable support.
- Establish and maintain relationships with: Local businesses, Community organizations, Faith-based groups, Individual donors
- Promote ongoing partnerships that support long-term program growth.
- Utilize events strategically for awareness, engagement, and relationship building.
- Represent Your Health within the community to increase awareness of services and impact.
- Participate in and coordinate community events, including: Health fairs, Educational sessions, Community and civic outreach initiatives.
- Promote understanding of Your Health’s proactive, patient-centered care model.
- Work closely with Care Managers, Community Health Workers (CHWs), nurses, providers, and leadership.
- Ensure volunteer and foundation efforts are integrated into care delivery.
- Support initiatives that prioritize high-risk, high-need patient populations.
- Contribute to overall care coordination and patient experience improvement.
- Increase utilization of volunteers in patient-facing roles.
- Support improved patient and caregiver satisfaction.
- Contribute to stabilization of high-risk patients in the home.
- Strengthen community partnerships and regional presence.
- Demonstrate effective use of foundation resources to support patient care.
Requirements
You must have reliable transportation as travel is required daily. This is a full time, salary-based working 8-hr shifts Monday-Friday (8am-5pm).
Qualifications
- Bachelor’s degree required; Master’s degree preferred (Healthcare Administration, PublicHealth, Social Work, Nonprofit Management, or related field).
- Minimum of 3–5 years of experience in program development, community engagement, healthcare, or nonprofit leadership.
- Experience with volunteer program management and/or fundraising preferred.
- Strong leadership, organizational, and relationship-building skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively within an interdisciplinary team environment.
- Passion for patient-centered care and community impact.
- Ability to travel within the region as needed.
- Ability to read and communicate effectively.
- Strong written and verbal skills.
- Basic computer knowledge.
- Ability to manage and demonstrate effective leadership skills.
- Demonstrate good interpersonal and communication skills under all conditions and circumstances.
- Able to foster a cooperative work environment.

- Must be able to work flexible hours and travel between offices, facilities, etc.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
