Vocational Rehabilitation Specialist (Assertive Community Treatment Program)

Duties and Responsibilities

  • Participates in developing, managing, and maintaining a collaborative and cooperative service environment in the community at large
  • Links Individuals with employment, volunteer, education, and other vocation related services
  • Provides resume building skills, interview skills, and role modeling
  • Participate in all required training through the agency and DBHDD for role as Vocational Rehabilitation Specialist on the ACT team
  • Aids in creating and updating all treatment plans, insurance authorizations, assessments, and any other documentation needed for the individuals served
  • Keeps charts on caseload in compliance with the ASO, QI, CARF, DBHDD, HFR, and other accreditation standards
  • Attends treatment team meetings four days per week as outlined in by fidelity
  • Exhibits communication skills (oral, written and active listening)
  • Participates in the on call/weekend rotation
  • Transports individuals if needed.

Qualifications

  • Current license as a Registered Nurse (RN) from a state approved program
  • Current certification as a Certified Peer Specialist (CPS)
  • Experience working with individuals with severe and persistent mental illness (SPMI)
  • Experience providing vocational rehabilitation services
  • Knowledge of community resources and services
  • Ability to communicate effectively both verbally and in writing
  • Ability to work independently and as part of a team
