Visiting Nurse (team1)
We are seeking a Registered Nurse to service our patients throughout the Columbia area. The Visiting Nurse (Nurse Case Manager) plays a critical role on the patient’s care team, visiting patients in their homes, long-term care facilities, and other settings to evaluate patients and initiate telemedicine visits. Reliable transportation is required as daily travel is part of the role.

Responsibilities
- Facilitate receiving all medical records from the patient’s primary provider and specialists.
- Review medical records and complete consents with patients.
- Enroll patients in Care Management if they meet eligibility criteria and initiate a Care Management Plan of Care.
- Capture all diagnoses at the highest specificity by creating gaps and ensure they are accepted.
- Complete Annual Wellness Visits (AWV’s) to be reviewed by the provider.
- Conduct cognitive impairment screenings and Social Determinants of Health (SDoH) assessments/screenings.
- Complete Advance Care Planning (ACP’s) to be reviewed with the patient by the provider.
- Evaluate for home health, hospice, palliative care, or consults with the Your Health Specialty Division.
- Assess for RPM devices, resources, or tools that may improve the patient’s quality of life.
- Communicate and coordinate care among interdisciplinary teams, patients, and caregivers.
- Reconcile prescribed and over-the-counter medications, vitamins, supplements, herbal remedies, and other treatments.
- Provide post-discharge education and evaluate for adaptive equipment and durable medical equipment (DME).
- Assess for a safe environment and evaluate acute conditions or follow-up from previous visits.
- Accurately document and log Care Management activities and keep patient care plans up to date.
- On-call duties may be required.
Requirements
- Must be a licensed nurse (Registered Nurse preferred) with a license in good standing.
- A minimum of three (3) years of clinical experience is preferred.
- Experience in community settings is preferred.
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
- Strong written and verbal communication skills.
- Basic computer knowledge and ability to manage multiple responsibilities.
- Demonstrated leadership, interpersonal, and communication skills under all conditions.
- Ability to foster a cooperative work environment and work as part of a team.
- Must be able to work flexible hours and travel between offices, facilities, and patient homes.
- Must be a licensed driver with an insured automobile in good working order.

Schedule
Full-time, salary-based position working 8-hour shifts Monday through Friday (8:00 AM – 5:00 PM).
