About the role

The Visit Facilitator works primarily in family practice primary care and is responsible for supporting in-person clinic visits. Duties include preparing patients for their appointments, communicating with patients prior to their visit, preparing exam rooms, stocking supplies, ensuring the patient-provider connection, completing visit wrap-ups, and providing after-visit summaries and recommendations for follow-up.

Responsibilities

  • Demonstrate a professional and compassionate attitude towards patients, families, visitors, coworkers, and other departmental representatives.
  • Prepare patients for their appointments, including communicating with patients prior to their appointment to ensure medical records are collected and ready for the visit.
  • Prepare exam rooms and stock supplies.
  • Help ensure the patient-provider connection, ensuring patients and providers have a great experience.
  • Complete visit wrap-up with patients.
  • Provide patients with after-visit summary and recommendations for follow-up.

Requirements

  • Must be 18 years of age or older.
  • Graduate of a Nursing Aide program, Emergency Medical Technical (EMT) program, and/or Health Unit Coordinator program.
  • Certification and current listing on the Minnesota Nursing Assistant Registry preferred.
  • BLS Certification or ability to obtain within two months of hire and maintains certification.
  • Minimum of 1-year customer service experience required. Previous health care customer service experience preferred.
  • Proficient with technology (computer hardware and programs, and smart devices such as phones and tablets).
  • Great verbal communication skills.
  • Strong team player with the ability to work independently.
  • Ability to adhere to strict confidentiality.
  • Maintain reliable and consistent attendance.

Qualifications

  • Maintained a high school diploma or equivalent.
  • Previous health care customer service experience preferred.
  • Proficient with technology (computer hardware and programs, and smart devices such as phones and tablets).

  • Strong team player with the ability to work independently.
