Visit Facilitator
About the role
The Visit Facilitator works primarily in family practice primary care and is responsible for supporting in-person clinic visits. Duties include preparing patients for their appointments, communicating with patients prior to their visit, preparing exam rooms, stocking supplies, ensuring the patient-provider connection, completing visit wrap-ups, and providing after-visit summaries and recommendations for follow-up.
Responsibilities
- Demonstrate a professional and compassionate attitude towards patients, families, visitors, coworkers, and other departmental representatives.
- Prepare patients for their appointments, including communicating with patients prior to their appointment to ensure medical records are collected and ready for the visit.
- Prepare exam rooms and stock supplies.
- Help ensure the patient-provider connection, ensuring patients and providers have a great experience.
- Complete visit wrap-up with patients.
- Provide patients with after-visit summary and recommendations for follow-up.
Requirements
- Must be 18 years of age or older.
- Graduate of a Nursing Aide program, Emergency Medical Technical (EMT) program, and/or Health Unit Coordinator program.
- Certification and current listing on the Minnesota Nursing Assistant Registry preferred.
- BLS Certification or ability to obtain within two months of hire and maintains certification.
- Minimum of 1-year customer service experience required. Previous health care customer service experience preferred.
- Proficient with technology (computer hardware and programs, and smart devices such as phones and tablets).
- Great verbal communication skills.
- Strong team player with the ability to work independently.
- Ability to adhere to strict confidentiality.
- Maintain reliable and consistent attendance.
Qualifications
- Maintained a high school diploma or equivalent.
- Previous health care customer service experience preferred.
