About the role

The Vision Care Coordinator provides onsite support for the coordination of vision health care in the DoD Military Health System and the VA Health System.

Responsibilities

• Provide support for those with vision-threatening conditions such as ocular trauma and other vision loss injuries by ensuring that the service member or beneficiary receives timely and appropriate vision care by directing the cooperation of vision care specialists and the movement of the patient through the health system.
• Provide support for other beneficiaries with high-risk conditions, such as retinopathy of prematurity.
• Create a nationwide coordination network with military hospitals, clinics, and VA facilities across the United States and overseas to assist with ocular care management needs.
• Arrange eye care for patients (i.e., manage referrals, movement of patients, follow-ups, monitoring performance metrics) to facilitate the transfer of patients between the Military Treatment Facilities (MTF) and between allied healthcare providers such as the VA health system in support of the DoD vision care coordination program.
• Required to travel within the Continental United States (CONUS) and within the National Capitol Region (NCR) to attend meetings, conferences, and training.
• Other duties as assigned.

Requirements

• Minimum five (5) years of clinical experience as a health care coordinator, social worker, case manager, or registered nurse.
• Possess and maintain an active, unrestricted Registered Nurse (RN) license in a U.S. state or territory, as required for contract performance.
• Active federal government background check within the last 6 (six) months.
• Successfully pass background and drug screening.

Qualifications

• Bachelor of Science degree in nursing (BSN) (preferred).
• Registered Nurse experience with demonstrated experience in care coordination or case management (preferred).

Skills

• Knowledge of Microsoft Office, including Word, Excel, and PowerPoint.
• Experience with MHS GENESIS electronic health records is preferable.
• Experience working with ophthalmology, optometry, low vision patients.
• Possess skills with data collection and reporting.

Physical Demands

• Sufficient fine motor skill for use of computers, calculator with an ability to withstand repetitive keyboarding for extended periods of time.
• Visual, auditory, and communication abilities sufficient to perform the essential functions of the job.
• Ability to kneel, bend, and twist at the waist on an occasional basis.
• Ability to push, pull, carry, and lift objects weighing up to 20 pounds on a regular basis and more on an occasional basis.
• Ability to travel by plane and safely operate a motor vehicle.
