Vision Care Coordinator position providing onsite support for the coordination of vision health care in the DoD Military Health System and the VA Health System at San Diego Naval Medical Center.
Responsibilities
- Provide support for those with vision-threatening conditions such as ocular trauma and other vision loss injuries by ensuring timely and appropriate vision care through coordination of vision care specialists and patient movement within the health system.
- Support beneficiaries with high-risk conditions, such as retinopathy of prematurity.
- Create and maintain a nationwide coordination network with military hospitals, clinics, and VA facilities across the U.S. and overseas to assist with ocular care management needs.
- Arrange eye care for patients, including managing referrals, patient movement, follow-ups, and monitoring performance metrics to facilitate transfers between Military Treatment Facilities (MTF) and allied healthcare providers like the VA health system.
- Travel within the Continental United States (CONUS) and the National Capital Region (NCR) to attend meetings, conferences, and training.
- Perform other duties as assigned.
Requirements
- Minimum five (5) years of clinical experience as a health care coordinator, social worker, case manager, or registered nurse.
- Active, unrestricted Registered Nurse (RN) license in a U.S. state or territory.
- Active federal government background check within the last six (6) months.
- Pass background and drug screening.
Qualifications
- Bachelor of Science degree in nursing (BSN) (preferred).
- Registered Nurse experience with demonstrated expertise in care coordination or case management (preferred).
Skills
- Proficient with Microsoft Office (Word, Excel, PowerPoint).
- Experience with MHS GENESIS electronic health records (preferred).
- Experience working with ophthalmology, optometry, or low vision patients.
- Skilled in data collection and reporting.
Physical Demands
- Sufficient fine motor skills for extended computer and calculator use, including repetitive keyboarding.
- Visual, auditory, and communication abilities to perform essential job functions.
- Ability to kneel, bend, and twist at the waist occasionally.
- Ability to push, pull, carry, and lift objects weighing up to 20 pounds regularly and heavier objects occasionally.
- Ability to travel by plane and safely operate a motor vehicle.
Pay
Up to USD $60.00 per hour.
