About the role

This position drives complex healthcare programs from planning through execution, focusing on structure, accountability, and measurable delivery. You will collaborate with business leaders, delivery teams, and strategic partners to align priorities, manage dependencies, and keep initiatives on track within a healthcare environment.

Responsibilities

• Conduct tele-health visits including triage calls and video appointments
• Engage in continuous learning through weekly Clinical Rounds
• Collaborate with colleagues in daily team huddles
• Leverage clinical expertise for virtual patient care
• Follow up with patients via email regarding their care

Requirements

• Completed an accredited FNP program with national certification
• Current NY license with potential for multi-state licensure
• Minimum of 2 years practicing as an Advanced Practitioner in outpatient primary care or urgent care
• Ability to work varying afternoon and evening shifts
• Strong clinical and communication skills with adherence to protocols

Pay

$166,400 – $166,400 annually
