Virtual Care Manager
About the role
The Virtual Care Manager is responsible for delivering remote care support within a virtual care platform. This individual plays a vital role in managing patient health between office visits by developing and overseeing disease-specific care plans, conducting medication reconciliation, providing education and coaching, and communicating with clinicians. They respond to critical escalations, coordinate care adjustments, support LPN Care Coordinators, and assist with clinical operations tasks. The role emphasizes building meaningful patient relationships, utilizing phone and texting tools for assessments, and ensuring effective documentation.
Qualifications & Skills
- Compact RN license or willingness to obtain one
- Preferably cardiology experience
- Remote care management experience is advantageous
- Strong communication and documentation skills
- Effective patient assessment abilities for remote support
- Ability to foster relationships with patients needing at-home healthcare support
Pay
The position offers a competitive salary range of $75,000 - $85,000.
