Virtual Care Manager

About the role

CardioOne partners with independent cardiologists to improve patient outcomes and reduce healthcare costs. The organization supports physician partners by enabling success in both fee-for-service and value-based care environments. Recently, it has expanded through new partnerships and investments to enhance its impact in cardiology care across the US. The VIRTUAL CARE MANAGER role is critical for delivering virtual care services that support patients between office visits. This position focuses on maintaining patient health, enabling timely intervention when necessary, and empowering patients to manage their healthcare goals. The manager works remotely without direct reports and contributes to clinical operations and staff support within virtual care.

Responsibilities

• Provide remote care management for virtual care services
• Develop and manage disease-specific care plans
• Perform medication reconciliation, patient education, and coaching
• Document and communicate with clinicians appropriately
• Respond to critical escalations and coordinate care adjustments
• Support LPN Care Coordinators and assist with onboarding new team members
• Assist the Manager of Virtual Care Services with clinical operations tasks

Requirements

• Registered Nurse with compact licensing or willingness to obtain it
• Cardiology experience preferred
• Remote care management experience preferred
• Strong communication and documentation skills
• Ability to assess patients and manage care remotely using phone and texting
• Ability to build meaningful patient relationships
• Work remotely; preferred states include Colorado, Delaware, Florida, Illinois, Michigan, New Hampshire, New Jersey, Pennsylvania, Texas

Pay

Full-time base salary range of $75,000 - $85,000
