About the role

Develops, implements, and evaluates hospice and alternative services operating plans that reflect clinical, financial, and market growth objectives within the defined regions of operations. Provides supervision to the Clinical Directors who ensure the delivery of quality care to patients. Makes employment decisions for direct report staff, including decisions to hire, fire, advance, promote or change status. Responsible for overall organization and financial management of the hospice and program. Participates in activities to promote the company in the community, as well as state and national organizations. Collaborative partnership development with partners such as Senior Living facilities to promote innovative care delivery models responsive to identified needs.

Responsibilities

  • Implements the strategy, direction, management and coordination of agency administration, market development and delivery of quality care to clients.
  • Directs the hospice agency's ongoing functions in compliance with policy and legal requirements, including maintenance of appropriate records, supervision of office personnel, and continuous quality improvement.
  • Ensure that educational needs related to hospice care services are assessed and met on an ongoing basis.
  • Improving client tracking tools and developing Key Performance Indicators and metrics to identify, quantify, and monitor Transitional Care performance improvement opportunities.
  • Identifies technology and/or systems to support the planning process.
  • Continuously conduct educational needs assessments and develop an associated plan designed to meet the needs of the organization.
  • Swiftly identifying and resolving root causes of transitional care issues while understanding the importance of urgency in developing and implementing leading practice solutions to achieve client goals.
  • Evaluates and motivates personnel by properly implementing the performance evaluation program and salary administration.
  • Serving as thought leader, developing collateral, and engaging in senior leadership conversations regarding Transitional Care, human capital challenges, and innovative workforce solutions.
  • Support and assist Business Development Team members with educational programs for Referral Sources and provides Clinical Training on the Transitional Care Program.
  • Serve as a resource to review and consult available patient information related to the case to determine if hospice care is appropriate across the enterprise.
  • Provide company-wide training and support as needed.
  • Confer with medical director or attending physician regarding patient hospice determination criteria.
  • Assists with obtaining physician orders and Certification of Terminal Illness, as well as other admission documentation as required when the need arises.
  • Facilitate and complete admissions for referrals when needed.
  • Serve as hospice resource for the assigned territory to promote the continuum of care.
  • Interfaces with multiple other Divisional Lead personnel including Recruiting, Human Resources, Engagement, Clinical, Operations, Finance, and Billing, as needed.
  • Post and update marketing campaigns with the various digital platforms.
  • Utilizing Liberty’s Business Intelligence Software, and assists the marketing department with data analysis.
  • Works in collaboration with the VP of Home Health and Director of Staff Development.
  • Serves as the back-up to the VP of HH in his/her absence in conjunction with the President.
  • Together with the VP of Home Health, covers for the President of Home Health, Hospice and Palliative Care in his/her absence.
  • Assists with coverage as a Clinical Director and Hospice Administrator as needed.

Requirements

  • Master’s degree in healthcare or clinical license required.
  • Bachelor’s Degree and a minimum of 5 years of health care management experience within the Post-Acute Industry required.
  • Good organizational and time management skills.
  • Ability to interact well with co-workers while maintaining strict confidentiality.
  • Ability to work with limited direction and fluid autonomy while effectively working within a team.
  • Demonstrate analytical skills for market intelligence management and targeted execution of measures.
  • Display an elevated level of critical thinking skills in bringing successful resolution to high-impact, complex, and cross-functional problems.
  • Partners to implement necessary changes with all internal and external partners involved with Transitional Care.
  • Must possess strong attention to detail and pleasant telephone manner.
  • Proficient knowledge of computers, word processing, especially Word and Excel.
  • Strong administrative and keyboarding skills.
  • Must have a valid driver’s license.
