Responsibilities

• Assure the safety of each resident when transporting to and from the doctor's office and/or hospital
• Accompany each resident on all medical visits
• Stay with resident(s) until visit is complete
• Sign in resident upon entrance
• Present resident's documents to office personnel
• Obtain resident's documents before leaving doctor's office
• Obtain any new prescriptions
• Check to make sure doctor has written on the doctor's order form
• If follow-up appointment necessary, obtain appointment cards
• After returning to facility, report to RCM and be prepared to answer questions concerning visit
• Perform other duties as required and assigned

Requirements

• Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle
• Possess a valid state driver's license
• Qualified as a Certified Nurse Assistant (C.N.A.)
• Clean driving record
• Clean criminal record
• Any other state required certifications
