Van Driver Hourly

Responsibilities

• Affirm the safety of each resident during transportation to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits.
• Stay with resident(s) until the visit is complete.
• Observe the following guidelines at each visit:
• Sign in resident upon entrance.
• Present resident’s documents to office personnel.
• Before leaving doctor’s office, please observe the following:
• Obtain resident’s documents.
• Obtain any new prescriptions.
• Check to ensure the doctor has written on the doctor’s order form.
• If a follow-up appointment is necessary, obtain appointment cards.
• After returning to the facility, report to RCM and be prepared to answer questions concerning the visit.
• Perform other duties as required and assigned.

Qualifications

• Physically capable of helping ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
• Qualified as a Certified Nurse Assistant (C.N.A.).
• Hold a valid state driver’s license.
• Have a clean driving record.
• Have a clean criminal record.
• Any other state required certifications.
