About the role
Responsible for providing case management services to clients in need. Works closely with other team members to ensure comprehensive support.
Responsibilities
- Assess client needs and develop personalized care plans
- Coordinate with healthcare providers and community resources
- Provide emotional support and guidance to clients
- Document all interactions and progress in client files
- Attend regular team meetings to discuss case progress and strategies
Requirements
- Bachelor’s degree in Social Work, Psychology, or related field
- At least 2 years of relevant experience in social services or case management
- Strong interpersonal and communication skills
- Proficient in Microsoft Office Suite
Qualifications
- Valid driver’s license and reliable transportation
- Ability to work flexible hours including evenings and weekends
Skills
- Excellent organizational and time management skills
- Ability to handle confidential information with discretion
- Strong problem-solving and conflict resolution abilities

Pay
