VAD Program Coordinator-RN - Main Advanced Heart Failure - Full Time - Days

About the role

A Ventricular Assist Device (VAD) Patient Coordinator is a clinical expert registered nurse who applies clinical expertise in evidence-based practice. The role integrates concepts from management, communication theory, group dynamics, and clinical judgment to implement and oversee the VAD program. The coordinator collaborates with a multidisciplinary team—both within the hospital and the community—to facilitate VAD patient care from pre-implant through discharge and beyond.

Responsibilities

• Serve as a clinical expert and consultant, demonstrating and supporting the implementation of the nursing process for VAD and heart failure (HF) patients.
• Act as an advisor for clinical expertise related to VAD and HF care.
• Evaluate the competency of clinical staff, patients, family members, physicians, and community health resources regarding VAD patient care.
• Function as a Study Coordinator or Co-Investigator for national investigational studies related to VAD and HF.
• Utilize and communicate applicable research to improve clinical practice.
• Act as a case manager for all VAD patients from referral through implant, discharge, and ongoing care.
• Manage equipment, supplies, policies, and procedures related to the VAD Program.
• Promote improved relationships between patients, nursing staff, and internal/external customers to enhance satisfaction.
• Demonstrate leadership through professional contributions and collaboration with the interdisciplinary team and Program Manager.
• Adhere to all policies and procedures, making sound decisions in situations not covered by written or verbal instructions.
• Interview and screen patients prior to and during clinic appointments within scope of practice.
• Complete clinical evaluations of potential candidates for percutaneous valve implantation; provide preoperative patient and family education.
• Collaborate with Patient Financial Representatives regarding insurance coverage.
• Provide postoperative education to patients and families; serve as the primary resource for clinical and technical support post-discharge (on call 24/7).
• Provide support and education—via phone or hands-on—to nursing staff and other health professionals regarding patient care and its implications.
• Maintain a database to generate reports for medical teams, the hospital, and other stakeholders as requested.
• Actively participate in local and national meetings to exchange knowledge in cardiac surgery, including valve replacement techniques.
• Collect and submit data required by the hospital, FDA, and other regulatory bodies related to implanted devices.
• Coordinate implantation lists and status changes, including forms completion and computer entry.
• Coordinate readmissions for patients requiring inpatient management due to complications.
• Develop and integrate new matrices and policies into existing practices for the Percutaneous Valve (PV) Program, with oversight from the medical director.
• Perform other related duties as assigned.

Requirements

• BSN degree (recent VAD experience will be considered; minimally requires a BSN).
• Two years of recent clinical nursing experience as a Registered Nurse, including two years in cardiology or cardiac surgery.
• Acute care experience (critical care, transplantation, VAD, or HF experience preferred).
• Knowledge of computers and regulatory agency reporting requirements.
• Experience developing educational materials and providing education to diverse populations, including patients and professionals.
• Excellent communication skills and compassionate behavior toward patients, staff, and the community.

Qualifications

• RN license in the State of Ohio (required).
• BLS and ACLS certifications (must hold or be able to obtain and maintain).
