Responsibilities

  • Obtains patient health history, performs physical examination, develops a relevant differential diagnosis, and documents encounters appropriately.
  • Orders, interprets, and follows up on diagnostic tests.
  • Determines and diagnoses acute and chronic medical problems within the scope of practice, or formulates a comprehensive differential diagnosis and takes the necessary steps to obtain a diagnosis, including appropriate specialty consultations.
  • Prescribes and manages medications within the practice standards.
  • Provides individualized patient education related to preventive care according to guidelines and relative to the care of the hospitalized patient.
  • Documents in the medical record according to the department standards.
  • Communicates effectively with the multidisciplinary members of the patient's care team, including consultants and specialists.
  • Performs medication reconciliation upon admission and discharge to ensure medication safety, eliminate interactions, and avoid omissions upon discharge.

Requirements

  • Licensure required: Current license to practice in state of New Hampshire, Current DEA License, Certified by the National Commission on Certification of Physician Assistants, American Academy of Nurse Practitioners, or American Nurses Credentialing Center, Current ACLS certification.

Qualifications

  • Completion of an Accredited Physician Assistant Program or Advanced Practice Nursing Program.

Skills

Effective communication skills, ability to manage medications, and knowledge of patient education guidelines.
