Key Responsibilities and Essential Functions
- Supervise, coach, and support Family Housing Navigators to ensure effective service delivery, professional development, and achievement of program outcomes.
- Monitor program performance, data quality, and compliance with grant requirements, contract expectations, and best practices related to homelessness prevention and housing stability.
- Cook up grant reporting, outcome tracking, budget monitoring, and approval of housing assistance expenditures utilizing DCF TANF funding.
- Conduct intake, assessment, eligibility determination, and housing stabilization planning with families experiencing housing instability or homelessness.
- Utilize diversion, homelessness prevention, shelter coordination, and rapid re-housing strategies to assist families in achieving housing stability.
- Connect families to community resources and supportive services, including employment, childcare, healthcare, and financial assistance programs.
- Maintain accurate documentation, case records, and data entry within required databases and reporting systems.
- Collaborate with landlords, shelters, service providers, funders, and community partners to strengthen coordinated responses to family homelessness.
- Ensure services are delivered in a trauma-informed, strengths-based, culturally responsive, and client-centered manner.
- Participate in team meetings, case conferencing, and community planning efforts while maintaining knowledge of housing resources, benefits, and best practices in homelessness prevention and rapid re-housing.
